We aim to offer services which are young people friendly. We welcome young people and aim to put them at ease when they come to the practice.

We have compiled some useful information which young people have told us is important to them when it comes to their health.

Health Checks

40 Plus Health Checks

Everyone aged 40-74 who does not have a pre-existing heart condition is eligible for a health check once every five years.  It is a free check-up of your overall health. It can tell you whether you’re at higher risk of getting certain health problems, such as:

During the check-up you’ll discuss how to reduce your risk of these conditions.

Annual health check Learning Disability and Autism

All patients aged 14 years and over with a learning disability and/or autism are eligible for an annual health check. An annual health check can help you stay well by talking to a doctor or nurse about your health and finding any problems early, so they can be sorted out.

You do not have to be ill to have a health check – in fact, most people have their annual health check when they are feeling well.

If you have a learning disability or care for somebody who does, make sure we are aware of this because not everyone may be on the practice’s Learning Disability register.  You can do this by telling our reception team, doctor or nurse.

Once on the register you will receive an invite each year to attend an appointment.

Carers health checks

The carers health check is the same as the NHS Health Check but is about looking after you as a Carer. It will focus on your own health and wellbeing and whether there are any areas you need support with. It is a chance to talk about your concerns and hopes for the future. The check covers the questions which Carers have told us are important.

Ex – Service Personnel Veterans

Our Practice Armed Forces Champion is Claire Baines. Our Armed Forces Champion will offer advice and support and will be able to signpost our Military Community to the services they may require.

Please let the practice know if you have served in the Armed Forces as a Regular, Reservist or Adult Volunteer Cadet.  We have a pack that you can collect from our reception.

The practice works with Sunderland Clinical Commissioning Group (SCCG) Armed Forces Leads and Veterans in Crisis in Sunderland to ensure those who have served or currently serving in the Military have acccess to information across the city on services and support that is available.

Smoking Cessation

The practice encourages patients to stop smoking and will signpost you to the available clinics. Please ask at reception if you are considering stopping smoking.

Flu & Pneumococcal Vaccination

This practice offers vaccination in line with Department of Health Guidance, flu and pneumococcal vaccinations are offered free to at risk patients including: The over 65s, patients who suffer from Heart Disease, Diabetes, COPD etc. The flu clinics usually run from September to December in the surgery. Pneumococcal vaccinations can be given at anytime, please ask the nurse for details.

Minor Surgery

The doctors can perform procedures within the Health Centre in the fully equipped minor surgery suite eg. Skin tag removal, excisions etc. Your GP will advise you should they consider a procedure necessary.

We also hold cyro clinics where our fully trained Practice Nurse removes warts, verruca’s etc.

Travel Injections

If you are travelling abroad you may require immunisations. Please make an appointment with the Practice Nurse approximately 8 weeks before you are due to travel. Most travel vaccines are free to our patients e.g Tetanus, Polio, Typhoid, Hepatitis A etc, but please check with us before making a booking.

Non-NHS Medical Examinations

We are happy to carry out any non-NHS medical examinations, e.g. Heavy Goods Vehicle, Pre-Employment etc. The receptionist will be able to advise you of the fee.

Named GP for Over 75’s and Well Persons Health Check

All patients over the age of 75 years will be allocated with a named GP who has overall responsibility for the care and support the surgery provides you. This does not prevent you from seeing any GP in the practice as you currenlty do. You will also be offered a free health assessment including: a urine test (Diabetes Screening), recording your Blood Pressure, Height, Weight, etc., please make an appointment with the Practice Nurse.

Chronic Disease Management, eg. Diabetic/Asthma/Heart Disease etc.

We provide comprehensive care for those suffering from chronic diseases, e.g. diabetes, asthma, coronary heart disease, etc. You will be called in for an annual review with the practice nurse, this will be in the month of your birthday.
