Online Access to Your Medical Records

You are currently able to view parts of your medical record securely online. For patients aged 16 years and over we can now enable access to the “problems” section of your records. This is where any health problems are recorded, for instance if you have been diagnosed with a medical condition such as Diabetes, Asthma etc.

Benefits

Being able to see your record online might help you to manage your medical conditions. It also means you can access it from anywhere in the world where you can access the internet, should you require medical treatment when you are away eg. on holiday or away on business.

Considerations

You are responsible for keeping access to this information safe, you should read the “Online Services Records Access Patient Information Leaflet” for other considerations. This leaflet is available in the surgery or for download in the “Our Documents” section of this website.

Non-urgent advice: Notice

To access your medical records you must read the above leaflet and complete the “Application for online access to my medical record” form, available in the surgery or for download in the “Our Documents” section of this website. You will be required to prove your identity by producing two documents including one picture ID (Passport; Driving licence) and proof of your current address (a utility bill, not older than 6 month’s old).
